You go in for your appointment and let the dentist do an exam and give his advice on what needs to be done; cleaning, x rays, treatment plan. Go from there, it will be in your best interest to follow the treatment plan to take care of any problems you have now, before they become major problems which can become very painful and expensive. The dentist won't do anything at this first appointment unless you agree to have the work done and he has the time to take care of it.
