Is midline worth it to extract 2 premolars?!

Is midline worth it to extract 2 premolars?

My orthodontist want me to remove 2 of my premolars to fix my midline. My midline is off by half of the front tooth. My teeths on the bottom are just slightly crowded but not alot. I need Advice!!!!

Answers:

Nancy is right! I'm a dentist and I do orthodontics. Please get a second opinion.

Extracting premolars is usually NOT the best way to fix a midline or a crowded arch. When upper premolars are extracted it usually means you have a jaw to jaw problem. In other words, your lower jaw is back too far and it probably looks like the problem is with your upper jaw (buck teeth, etc.) By extracting upper premolars and pulling upper teeth back to close the spaces created by the extractions and make the upper jaw fit the lower jaw (that is too far back) now makes the upper jaw too far back too.

Your upper teeth give your upper lip support, when you pull the upper teeth back your upper lip goes with the teeth. This gives your face a flat appearance and makes your nose look pointed and long. When looking at the face from a profile it looks like an arrow. Your teeth might look nice and straight but your face will be compromised.

Find a dentist/orthodontist who does both "fixed AND functional orthodontics." You most assuredly need to have your lower jaw brought out to match your upper jaw. This can only be accomplished in two ways: surgery or with functional orthodontics. I don't recommend surgery but rather functional orthodontic appliances.

Look into it and make this decision very carefully. I've seen pictures of identical twins, one had the treatment your orthodontist is recommending, the other had the treatment I'm recommending. When they were finished with treatment you could hardly even tell the twins were related, much less that they were identical twins. And guess who had the more attractive face?

Hope this helps.
