Answers:

Okay... let me break it down for you...
Maxilarry means your upper teeth, mandibular means your lower.
Left and right are exactly what you'd think.
1st bicuspids... Well... in each quadrant (quarter) of your mouth, you POTENTIALLY have 8 teeth: 2 incisors, a canine, 2 bicuspids and 3 molars (including your wisdom teeth.)
If you don't have wisdom teeth, and you have two molars, then your 1st bicuspid will be TWO teeth in front of your molars. They kind of look like a smaller molar, and they're called "bicuspids" because they have two (bi) cusps (peaks, similar to mountain peaks)
I hope that this makes sense...
So, again assuming you have no wisdom teeth and 2 molars, you're having the 4th tooth from the very back of your mouth pulled on both sides, upper and lower.
Are you getting braces by chance? These are common teeth to have pulled if you're getting braces.
And no, they most likely will not put you to sleep. They'll give you injections and then they'll pull the teeth. You won't feel anything except a little pressure. It's not bad at all. You'll be completely surprised when they tell you its over!
