Cause of toothache in upper molar that had a root canal?

molar is right upper and has dead nerve. No evidence of cavity on x-ray but covered with crown. Occurs approx. every 6 months for several years and lasts about 10 days with or without antibiotics.

Sometimes new trauma,deep decay or a loose,cracked or broken filling can cause new infection in your tooth. Endodontic surgery may be able to save the tooth.They usually do a procedure called a apicoectomy or root resection.A small incison is made in the gum tissue near the tooth to expose the bone and infected tissue. The infected tissue is removed and the tip of the root is also removed.Sometimes a small filling is placed to seal the root canal.
