You probably had a tooth-colored filling (composite) put in there. It sounds like you were charged for a three-surface filling when all you had was one... ask what surfaces were involved with the filling.

Posterior teeth have the following surfaces: Mesial, Distal, Occlusal, Buccal and Lingual..... there are 5, not 4.

it means you had a rather large filling put in. the tooth has four surfaces, you had to have three surfaces filled.
