It shouldn't hurt at all. It feels more like a pressure along your nose region, but it goes away rather quickly. It's an adjustment to swallow because the tongue goes to the roof of the mouth when we swallow. It has to retrain itself since the palate is filled with your appliance. Food does tend to get stuck, but that's only because you haven't learned how to fully chew and swallow yet. If you can, get a waterpick or use warm water to gargle after you eat. It will remove food debris pretty easily. It will take a day or two to get used to it. You will salivate more than normal (your body still sees it as foreign like food). You may have speech difficulties, but this will lesson the more you speak. Read a book aloud or talk on the phone with a friend. The more you speak, the faster you will adjust. I don't think anything hurts as badly as separators, and I've had braces twice...once as a child and once as an adult. I'm an orthodontic assistant, and my daughter has had expanders. She never complained of pain, only pressure and it never lasted longer than a few minutes. I've not had any patients tell me that expanders hurt with the exception of those who had jaw surgery to expand their palates (the suture had to be broken so it could be expanded).

you should feel pressure while tightening it but thats it.
its going to be hard to swallow the first week, but after that you'll slowly be able to get food down.
