I don't know if you can. Yes, these are the two most popular ingredients claimed to be in today's toothpastes. But making it yourself...baking soda fizzes and dissolves when wet and hydrogen peroxide fizzes and becomes water + oxygen when exposed to certain catalysts. That's why it bubbles when you use it to clean a bleeding wound. I would suppose that if you added just a little peroxide at a time to the bicarb until you get a paste, much like I add just a spritz of witch hazel extract to bicarb when I make bath seltzers, then it wouldn't set off the reaction. You'll have to try.
Let me know, I can't seem to find a toothpaste that isn't some flavor of mint. I hate mint, so I want to make my own toothpaste
