Ache in lateral incisors, three out of four. No explanation.?!

Question:

Ache in lateral incisors, three out of four. No explanation.?

One is on top left next to bicuspid that was root canaled in October 2006 and has not been capped yet (waiting for insurance in July), while the other is opposite that and is next to a bicuspid that had a tiny filling done in April 2007 which previously caused a stinging feeling, the one under that started aching in February 2007. Everyday the one next to the filled one still aches, while some days the other two ache more or less. Every night I wear a bruxism guard. Advil doesn't help much, while an ice pack inserted into mouth helps some. No avoidance or combination of food makes any difference from all soft to crunchy with hot peppers. I chew on both sides of my mouth. I've taken antibiotics for 3 weeks with only slight improvement. I have had a full face X-Ray and it showed no sinus infection 100% clear. Why do my teeth still ache after 6 months?

Because the three dentists and one physician I've seen have offered me no advice.

Answers:

It could be your age, how old are you?
It could be your teeth positioning in reference to eachother, have you had braces?
It could be that you grind your teeth when you sleep, Have you asked your dentist about TMJ?
