Hi! I need to have work done on my teeth asap from a dentist, but recently my wallet got stolen and i lost my drivers license/social security card/credit cards/etc. I'm planning on going to a dentist next week (i currently dont have one), i know they make you fill out a form for new customers, i was wondering if they ask for ss number, drivers license, etc? thanks

If you have insurance then yes they will ask for ss # and in our office we ask for a copy of your drivers license. Not all do that though.
If you pay cash then there should be no need to give them your ss #. If you pay with credit then they'll probably want to see some id. If it's an office policy then you're out of luck.
It's basically if they have to send you to collections.
