My reply would be that for some, the dentist would say it is not that bad. Then tell the person to use a fluoride mouthwash or use a specific type of tooth past so the condition does not get worse. Others drill every hole they can to rack up the bill on the insurance claim.

I don't expect a cavity can get better with time, unless of course it happens to be infected. In that case the infection could be cleared up with medication, but the cavity will remain untill filled or the tooth is pulled.

They don't get better by themselves, but they grow at different rates. Sometimes a dentist will see one and think it's not serious enough to fix right away. I think it depends on whether the dentist just bought a new Lexus. 8^)

If the dentist doesn't do a filling, it means it's not a cavity yet (it's not yet through the enamel layer). At this point, using fluoride toothpaste can strengthen it and it may never become a cavity (or it may, if you don't take care of it).

If it is through the enamel layer, it is a cavity, and it will not get better. The inside of the tooth will start to decay at this point.

If it is not filled, it will get much worse and you will eventually need either a root canal or the tooth will need to be pulled.

Basically, if the dentist doesn't fill it, it's not a cavity yet, it's called a cavity watch. If they do a filling it's because you have a cavity and need it filled.
