If the tooth is dead and ready to fall out, you could pull it out at home ( I caution you to be careful though because if it's still living and just rotting or loose you could have a LOT of pain in store because of an exposed nerve).
Now, if you do decide to pull it, the tool to use depends on which tooth it is. If it's one of the bigger teeth in back, you can probably use a regular pliers - more surface area to get a good grip. If it's a smaller tooth, use the needle nose to get a good grip (make sure you have a good grip on the RIGHT tooth before you yank too!!).
I've done it. A couple years ago with a tooth that was dead. There was little to no pain once I got it out. I use a regular pliers because it was a large tooth and I was able to get a good grip on it that way.
CAUTION: Have a bottle of your favorite hard liquor, some ambisol (not sure how to spell it), and a person you trust to stay calm standing by just in case something goes wrong.
