Answers:

Depends on what state you are practicing. A majority of states would require the obvious, dental malpractice insurance, some form of workers compensation premium (if you have people working for you), and possibly a bond. It would be a good idea to pick up insurance on the following as well: building/office (even if you rent), damage, umbrella (for large claims) and some life insurnace on your partner, if any.
