Every dentist, specialist and physician will reject a case that they feel is outside the scope of their skillset. If your orthodontist refused to perform the surgery, it's either that he/she feels you don't need it or they are not qualified to perform it. Get a second, and maybe a third, consult with other doctors. When considering something as major as jaw surgery it never hurts to have a couple of opinions.

I wouldn't have surgery done by an orthodontist, it should be done by an oral surgeon. If you're talking about TMJ surgery, it's usually not very successful anyway.

not very often. but that is a good thing. it means your teeth are too healthy for a surgery 8)

Orthodontics do not perform surgery. They would work the case with the oral surgeon if the case involve orthodontics treatment and surgery. You should go to another orthodontic and if he/she do not want to work with your case ask them for a referral to another orthodontic. If you think you need surgery you can go for a consultation with an oral surgeon, they also know doctor of orthodontics that can help you.
