Okay, you've put so much into this question it's hard to tell what issue it is you're having!.

First, if I'm reading your post correctly you just had five large fillings and a post/core/crown!. That means you're having fairly extensive dental work done!. With multiple changes in the mouth it can take a couple of follow-up visits to address any minor concerns that arise!. I suggest you go back to the dentist who did the work!. Bring up all your concerns!. None of it sounds terribly unusual!.

Here's my take on your list: the bite being off, very common!. When you are numb it's almost impossible for you to bite correctly so it's often necessary to come back the next day when you are no longer numb for an adjustment!. It's a painless fix and you need to get the adjustment!. Leaving it can result in other problems!.

The sore gums, probably related to the band placement when the fillings were done!. A band is needed for the surfaces you had filled and it may have cut your gum!.!.!. not the doc's fault just a byproduct of the procedure!. Could also be irritated from your bite being high if it's in that same area!.

The little hole on the inside sounds like a pit or may even be just flash left over from the fillings!. Again, usually an easy fix by the doc!.

The little bit of black, could be some left over metal filling material if you had a filling there previously or some blood under the filling or it could be just stain in a tooth groove unrelated to the filling on the tooth!. That you need to get checked!.

As for the gum trauma, it's unlikely that the tenderness of your gums is related to a six year old trauma unless there is an abscess developing!. Since the doc just took x-rays a that doesn't seem likely!. It's more probable that you have periodontal disease developing!. Are you flossing daily!?

Basically, you just invested $3000 in treatment!. Go back to your dentist and ask about each issue!. Most of them sound like minor fixes and would be considered normal for the treatment you received and likely won't result in any additional costs to you!. If, after consulting the original dentist, you still have concerns then consider getting a second opinion!. You have far too many questions to rely on yahoo answers for treatment advise!. You need a clinical diagnosis performed by a licensed professional who has all your records available for reference!.
