It's not possible to say what exactly is wrong, but I can guarantee you that eventually you will need to see your dentist for treatment when this becomes painful!. If taking an antibiotic seems to help and there is swelling, along with occasional pain, then this is a clear indication of an infection!. You may have something stuck under the tissue, a perio abscess or the tooth it's self may be abscessing!. In any case, only your dentist can examine, x ray, and treat this!. You will need to be seen and placed on an antibiotic therapy prior to any procedure so that the anesthetic will be effective!. Obviously the last time you were anesthetized, there must have been an infection present that blocked the anesthetic!. See your dentist soon before this becomes very painful and dangerous!. Good luck!.

Additional information: If this has been going "off and on" for several months, then it's probably decay that has reached the nerve, or some trauma that has effected the nerve of the tooth causing the tooth to die!. The abscess or swelling is the result of the pus building up at the apex of the tooth!. I would suggest you see your dentist ASAP before this tooth really flares up causing you a great deal of pain!.

Additional information: "Sara B" below, may be correct, if this swelling is on "top of the gum tissue, behind the second molar!." From what you described it sounded like it was "on" the second molars tissue surrounding the tooth's root!. If this is the case you will need to see your dentist to have the 3rd molar evaluated; there may not be enough room for the eruption!. In most cases we prefer to remove the 3rd molar; they are difficult to keep clean causing decay to develop, constant tissue infections or periocoronitis and crowding of the teeth!. Best to see your dentist!.Www@Answer-Health@Com

You either have a gum infection, need a root canal or have the tooth pulled!.

Is it on the upper or lower!? Upper could mean that you have a sinus infection too!.

Either upper or lower!.!.!.!.!.!.you could have gotten food stuck into the gum!.!.!.!.!.!.like a chip or something hard and it gets infected!.!.!.!.!.!.!.!.try flossing!.!.!.!.!.!.!.!.water picking and swishing with warm water!.!.!.!.!.!.

See an Endodontist or an Oral Surgeon!.!.!.!.!.if it is still painful!. Before they can do any work on the area you will have to take more antibiotics!.!.!.!.!.!.!.and even at that it might not clear it up as the shots don't numb the area if it is infected!

Chances are, you have just irritated your gums with something you ate - maybe a chip with a sharp edge or popcorn!. Whatever food injured your gum could be stuck in your gum, thus causing continued irritation!. Go see your dentist!Www@Answer-Health@Com

If the swelling is on your gums and its a lower molar and you are 20 years old based on what you wrote!.!.!.i think it is just your wisdom tooth coming in!.!.!.Www@Answer-Health@Com

Sounds like an abscess, I had one very similar to that a few years ago, you need to get to a Dentist right away!.
If left untreated, it can cause general infection, the loss of the tooth, or worse, including general infection of the bone and massive tooth loss or spread into other parts of the body!.
Not all abcesses are painful, so pain is not a good indicator!. But the the swelling you described, and especially the fact that penicillin helps, are very much signs of one!.Www@Answer-Health@Com
