Question: To get free prescriptions and dental care when not working, what forms do I need!?
Just wondering how to claim free prescriptions etc when you'reon the dole!.Www@Answer-Health@Com


Answers:
If you visit the dentist and need treatment/prescriptions you need to be claiming the following!.
Income support
income based job seekers allowance
pension credit
have had a baby in the last 12 months or are pregnant
if you are under 18 or are 18 and in full time education
If you are in a young offenders institute!
If you are any of the above then you are entitled to free NHS dental treatment and prescriptions!.
You will have to bring some form of proof of your benefit, as you have to sign an FP17 form and the dentist has to declare that he/she has seen your proof, your national number will not be classed as proof of exemption!.Www@Answer-Health@Com

You don't need extra ID, tell your dentist and you then sign a form stating you are exempt, the same goes for prescriptions just fill out the back stating the reason you are exempt!.Www@Answer-Health@Com

For prescriptions just sign the back, some dentists will ask for proof of eligibility, just show your signing on card or give your national insurance number!.Www@Answer-Health@Com

You sign a declaration at the dentist or on the perscription!.Www@Answer-Health@Com
