dear patient due to our numerous attempts to collect your outstanding debt we believe it is in the best interest of both parties that our doctor patient relationship be terminated at this time!. please be advised that you have a thirty day time frame in which to obtain another dental provider!.please contact are office if any further questions or concerns arise!.thank you!.!.!.Www@Answer-Health@Com
Keep it Simple!.
This letter is to inform you of your final notice of delinquency in paying the amount owed for services rendered by (your office)!. If not paid within 30 days time from postmark your name will be sent to a collections agency and we will respectfully decline your continued business!. After this point, we assume no further responsibility in assisting with your dental health and well being!.

I would simply tell them that their account has been sent to collections due to a breach in the financial responsibility (I am sure they signed some kind of form when they began treatment!.!.!.) and also say that because it has become a collection account, you can no longer treat them at your facility but would be happy to forward their charts and history on to their new dental office!.Www@Answer-Health@Com
