Answers:
1!. "JuliaT" gave you an excellent list ! (though she forgot the curing light!. Don't forget the sleeve cover that goes over the tip and the barrier tape on the handle to follow the OSHA protocol!. Some prefer to use the eye protector with it!.)

a) The name of the interproximal retainer that I use for the interproximal areas is "Compositite"!. There are other brands available!. This is shaped like a small metal wing!. It comes with a ring!. I use these for the interproximal areas of the posterior teeth!.

b) Most of the time though you will need "Mylar Strip" (thin plastic strip) that you will have to use so that the teeth do not get stuck together!.

You will need wedges to make both a & b adapt properly along the gingival area!. Otherwise, you will get huge overhang and subsequent leakage!.

2!. Also, "deputyda!.!.!.!." mentioned the etchant, anesthetic, and the gel!.

Don't forget the syringe with that!.

3!. You need the brush with small tip and a well for the bonding agent!. No need for a well if you are using the individual capsules!.

NOTE :

1!. Excavator (and explorer) is a must!. If you do not make sure all the decay area has been cleaned out well, might as well not perform the procedure!.

2!. Some dentists use "Snoop" to detect residual decay!.

3!. Make sure you keep "Vitrebond" or similar dental base material (Glass Ionomer) ready at hand for 'deep' preparations!. A cavity that is too deep may also require "Dycal" (Calcium Hydroxide) underneath the "Vitrebond"!.

Remember, you CANNOT use "IRM" underneath this type of filling because it has Eugenol in it, which will compromise the bond and integrity of the composite material!.Www@Answer-Health@Com

Excavator, but it's not necessary for a composite filling!. It's a long time since I was a Dental Nurse but the preparation of the cavity was the same as for an amalgam filling as in drilling out the decay and lining!. The difference was in mixing the 'white filling' powder with a glass spatula on a glass block!. The light cured composite fillings were just arriving and required only a smoother, eye protector for the patient and the curing light!. Burnishers were used more for amalgam than composite but maybe that was the Dentists preference!.Www@Answer-Health@Com

High speed suction
Saliva Ejectors
Mirror (sometimes I put out an extra one in case I have to retract the tongue!.)
Explorer
Excavator (spoon)
Condenser
Composite Instrument
You may need retraction cord if it is a class 5 (gumline) caries
Inteproximal bands and reatiner (there is a name for these and for the life of me I cannot think it right now) so if the caries goes into the mesial or distal surface!.
Cotton rolls
2X2's
High speed Handpiece
Slow speed handpiece
Caries and finishing burs
Composite polishing paste and wheel
Articulating forceps and paper
Patient Napkin and Bib
Chain for Bib
Anything else preferred by your dentist!.!.!.as all dentist are different and are trained to do the procedures different!.Www@Answer-Health@Com

Don't forget the topical and the anesthetic!
I would not think a dentist would use and elevator(if that was what you meant),as that is a surgical instrument!.
Also, a bonding agent is needed to adhere the composite to the tooth and I don't recall seeing an etchant, you'll need that as well!.!.!.!.
As far as what is needed, all that has been mentioned and whatever else the doc wants- they all have their own special twist on things!.Www@Answer-Health@Com
