A healthy nerve in a tooth will respond to a very cold stimulus (a stimulus similar to eating ice cream, but colder)!. If there was no such response to extreme cold, the nerve is probably dead!. An x-ray will often confirm this conclusion!. If you doubt your dentist's diagnosis get a second opinion!.

Before the nerve dies it can be hypersensitive to cold!. In that case, a slight cold stimulus, such as cold water from a drinking fountain or ice in the mouth, will cause acute, lingering pain!. After the nerve dies, there is no longer any response to cold (even to the extreme cold test that the dentist does)!. But the tooth may be painful to chewing because the supporting tissues around the tooth are inflamed!.

If the nerve is dead you should have root canal therapy to prevent infection and help keep your tooth!. You may indeed also be grinding your teeth at night, which is contributing to your symptoms, but it is probably not the central cause!.
