That probably depends on your doctor and whether you talk to him about it. Mine always did but it costs extra.
Probably not, they'll probably give you a couple shots of Novocaine to numb your mouth. I had to get Novocaine in my mouth for a similar procedure, and it didn't hurt at all because there's a lot of tissue in your mouth. You'll be ok, don't worry. Good luck!
No you won't be sedated. It's not bad though. Also you aren't getting a tendon cut, you are having your gingival crest fibers severed to enable rotational movement. Tendons connect muscles to bones.

Getting put out is risky, but to be honest the shot can range from barley feeling anything to a 5 out of 1-10 on the pain scale. I used to be TERRIFIED of needles in my mouth. I got over it so can you trust me!
Tell them you want to be put out. Many dentists are offering this now. Easier for them also. The shot should never hurt. Sign of a bad dentist.
