What you need is a new filling which has no microleakage points and proper insulation which is a cement lining given before the filling is placed. The white filling you have right now is most probably a temporar restoration.. Time to get it permanent, yes its the silver one.

I would avoid metal fillings. They are very aggresive, meaning you remove a lot of healthy tooth structure for them to stay in place. A tooth colored filling is bonded in place. So they can be very small. Metal fillings are ugly too. A white filling when done correctly will not be sensitive. If your dentist is inexperienced placing white fillings he/she will blame the material. Composite fillings have been around since the 1970s. They continue to get better and better.

Ideally, you can have the filling removed and a temporary filling placed. If the tooth stops hurting, then it was because the filling was bonded inproperly. If the pain continues, then you most likely have a tooth fracture or need a root canal because the nerve is inflammed.

If you need a root canal, the tooth will be sensitive to hot and cold that lingers for several minutes. If the tooth has spontaneous pain, you need a root canal. If you need a root canal, go to an endodontist because that is their specialty and that is all they do everyday. If you have a root canal, then your general dentist needs to place a cap or crown on the tooth to protect it. The root canal procedure removes the nerve and blood supply of the tooth. By removing the nerve and blood supply, the tooth essentially is non-vital or dead. A root canaled tooth then becomes very brittle and susceptible to fracture hence the reason for the crown.

Now if the tooth hurts to bite on with the temporary filling, then you most likely have a cracked tooth and need a crown or cap on the tooth. The crown will stop the tooth from flexing by splinting the fracture.

If the pain subsides with the temporary filling, then you need a new filling placed by a more experienced dentist and avoid the metal.

Hope this helps.
