It doesn't matter how much you brush. You can still have bad teeth. I always brush and take care of my teeth and I always have cavities. It's just weak teeth. My brother never brushed his teeth growing up and he never had cavities, go figure.
Get to a dentist. If you don't have insurance call your local welfare office and ask them if they know of any programs that go by income. My local hospital has a dental clinic and I only pay $50 a visit (compared to almost $200). I hate the dentist, but your teeth will only get worse.

It sounds like you have tooth decay, which could be due to the food you are eating, sometimes if you eat something hard, this can happen, BUT also, if you eat too many things such as chocolates and sweets, and drink too many fizzy drinks this can cause your teeth to rot over time.
You should try to clean your teeth for 2minutes whilst brushing, but do not brush too hard. It is adviseable that you use Mouth wash in the morning after cleaning your teeth to remove more plaque and give the teeth extra protection.
I would recommend start brushing your teeth atleast 3 times aday, In the morning, After lunch(If possible) (OR After dinner), AND at night before bed.
I would recommend you going into your dentist to get those teeth sorted. They will just simply use filling to fill the gap up. This must be sorted out as soon as possible before it leads to you gettin serious tooth ache, and perhaps having to have one or more of your teeth removed.
Also you should Floss your teeth every once in awhile :]

First of all, do you floss? Brushing only does so much, but it is the plaque in between the teeth that predominantly causes cavities and gum disease. You definitely have tooth decay, which is why pieces of your teeth are falling off. Get this taken care of before it turns into a serious problem and you have to get a root canal.... or else you might just lose that tooth altogether and it will be a very painful process, trust me. After you get it fixed, or even if you choose not to, floss from now on, aim for twice a day but at least once is fine. Flossing is almost more important than brushing because again, that's where plaque gets stuck and rots your teeth. You could brush your teeth for an hour but it still won't reach where floss does.
So please, REMEMBER TO FLOSS!!!! Also, listerine makes a enamel strengthening mouth wash, that can help too, but please, flossing is the most important factor here. Try to floss and brush your teeth after you eat or at least twice a day. My dad is a dentist, so please take my word for it.
