It sounds like you've been taking good care of you teeth, but perhaps you should look at your diet. Sugars can create bad breath quite quickly. They can come from just about any carbohydrate source, including milk and other dairy products. Start keeping a journal of what you eat and when you develop bad breath. You should see a correlation quickly.
In the meantime, it doesn't hurt to brush your teeth often and rinse frequently. Make sure to wait 30 minutes after brushing and/or rinsing before eating or drinking anything. And keep a supply of sugar free mints or gum nearby for when you start feeling the halitosis starting.

If not that, do see your dentist. You may have an infection somewhere.

You may have a gum problem you should ask your dentist and make sure you are brushing your tongue as well
Brush your teeth twice a day and rinse your face after you eat anything
