How do mouth ulcers originate what do they look like?!

My boyfriend keeps getting them on his tongue and inside his cheeks. I only had one once...he says his tongue is sore sometimes and he also says his lip is sore... what could be the reason for this? could it be because it is could outside or maybe even an STD? we've both been checked and dont have any STD's but is there a possility at all?
