personal details form for dentist?!


Question: Personal details form for dentist?
do i have to fill this form in for my dentist,it asks all personal details medical history medication i am on there is about nothing the form it dont ask,the form has no title or number,it was given to me yesturday when i was making a appointment at my dentist,i dont feel comfortable filling it in but do i have to to be able to see the dentist surgon,this is for essex U.K

Hi there, yes all NHS dentists and Private Dentists require a Medical History form completed for any patient they see. In my clinic we need our patients to complete one every 3 months or if nothing has changed since the last time they must sign it and state that. The dentist and nurse must me aware of your medical history for your safety and theirs. This is a confidential record and no one gets access to this at any time, your GP has the same information in your doctors clinic. All medical history forms are similar and should ask the following -
NAME, ADDRESS, DOB, AGE, PREGNANT OR NOT, BREASTFEEDING OR NOT, CURRENT MEDICATIONS TAKING, ANY PREVIOUS ILLNESSES, ANY CURRENT ILLNESSES, EVER BEEN IN HOSPITAL AND WHY, IF HIV OR HEP POSITIVE, IF HAVE EPILEPSY, DIABETES, HEART DISEASE, BLEEDING ISSUES, BLOOD CLOTTING ISSUES, EVER TAKEN RECREATIONAL DRUGS, HIGH OR LOW BLOOD PRESSURE , ETC. Your dentists may differ but that is what is on mine. The above are for information and very important as if you are prone to bleed excessively and dont tell the dentist you could bleed and they would not know why, and they would not have been prepared wih the correct meds to stop it or if you are on certain medications for a medical condition and have a reaction on the dental chair they would not know why. Also if a patient had a blood borne virus such as HEP C the staff need to know just in case they have a needle stick injury etc. It is something you should not feel uncomfortable about as if you had you hands and fingers in someones mouth you need trust and honesty there.
