How do you read this eyeglass prescription?!
Question: OD +1.75-1.00*005
OS +1.50 DS
Near Add: +2.25
Namely, what I put for the OD-SPH, OD-CYL, OD-AXIS, OD-ADD
and the OS-SPH, OS-CYL, OS-AXIS, OS ADD.
I wanted to order prescription glasses online but I couldn't figure out where the numbers go.
Thank you.

You will also need to know the distance between your eyes called a pupil measurement and depending on if you are getting a lined bifocal or no line your measurements need to be accurate.
you would be better off to go to a qualified optometrist, have the prescription filled, and have the glasses (or lenses) fitted properly. You may save money on line, but you only get one pair of eyes. It pays to do it right the first time.
