I think (emphasize think, I don't know for sure) that it has to do with being able to pick out words amongst meaningless noise, seeing if comprehension of what is being said is effected by its presence. Some people who are hard of hearing have particular trouble not so much with the actual "hearing," but rather with recognizing and interpreting sounds that carry meaning, ex. usually words. Basically they might know they are hearing someone say words, they are just not sure what is being specifically said.
Its just a hearing test, sounds like his hearing is becoming questionable, or his understanding of words if failing.
