This sounds like a rash of some kind. If it feels like a sunburn, even if it is cold outside, if you are bundled so much that that your skin can't breath then it could be a heat rash(1). You could have an allergy to the clothing that u wear when it is cold. If all it is is a rash, the Doctor will most likely give u some sort of cream to care for it. But then u talk about a sharp pain (not generally associated with rashes or sunburn), that sounds like a problem with your nerves (pinched). Even though a Chiropractor will be able to adjust your back if it is a pinched nerve, you will probably want to see your doctor first, in case it is shingles or something like that (also very sensitive) (2). The more detailed u are with symptoms the easier it is to diagnose yourself at say webMD. Diagnose yourself, then talk to your doctor, this helps them tremendously (sometimes they look over possibilities because u don't fit in a bracket...example, you have colon cancer but you are only 28 years old, usually doctors don't check on that stuff until you are 40, and yet younger people get it all the time) Hope that helps!!!

Most of the time, if you go to a general doctor, he will probably prescibe some type of drugs. If you go to a chiropractor, he will take an xray and adjust your back. If you go to an orthepedic doctor, he will probably have you do some exercises and physical therapy with massages and stretches. It really depends on who you go to see for your problems.
