A transsexual man may be able to have an orgasm, depending on which technique he opts for to create his penis; if he has a metoidoplasty, or a combination metoidoplasty/phalloplasty, he should be able to; the nerve supply is left intact. If he opts for pedicle-flap phalloplasty, he will have less success; the tissue used is not usually well-supplied with nerve endings, so it's not particularly sensitive.

A transsexual woman is luckier, in this respect; the nerve supply to her new clitoris is undamaged, and she also has some sensation in the lining of her vagina (slightly more than most other women), so she has a much higher (80-95%) chance of achieving orgasm.

In either case, it can take several months (sometimes up to a year) for the nerves to recover from surgery; it's unusual for transsexual people of either gender to manage orgasm in the first few weeks/months after surgery.

Some studies say that 80 to 100 percent of male-to-female transsexuals experience orgasm after sex reassignment surgery (this term is preferred to "sex change operation" nowadays). Dr. Biber claims a 95 percent orgasm rate in patients age 18 to 35, which is higher than the rate for biological women. Typically the researchers take the respondents' word for it, though. One suspects a certain amount of wishful thinking.
