You need to tell someone - this is one time that your deception to your friend is okay. Talk to someone you trust - your mom, her mom, guidance counselor - anyone can help. Just because she looks like she has a perfect life on the outside, does not mean she has a perfect life behind closed doors or on the inside. She could have family probs, mental probs, school probs - it could be anything. It's not up to you to decide whether it's severe enough to tell - just tell and let a professional take care of it. Even if she is mad at you at first, she will forgive you when she gets better. Do it out of friendship.
She sounds like she is crying out for attention. This could be a mental disorder that needs addressed. If you are really worried about her then you need to tell someone that can get her some help.
