Question: Yes it does. Unfortunately what keeps your urine in is the body contracting it's muscles to contain the contents. When the body expires, and there is no more conscious control, all the muscles relax. This in turn empties the bladder and the contents of the bowel.

Yes I believe you lose all bladder and bowel control. I'm 100% certain about the loss of bowel though.

Often is does, but not always. It partly depends on the person's dying process. In the event of traumatic injury leading to death, it is more likely to lead to incontinence (loss of urine or bowel). A person who has been sick for a while may not be incontinent as they their bowel or kidneys may not be producing waste, or they may not have been eating/drinking for a few days.
