Hiccups are actually a spasm of your diaphragm.
It is usually caused by air entering or trapping itself in your system in an awkward way. Usually a rush of air into the lungs will cause the epiglottis to close, and that is what makes the 'hiccup" sound.

It is your body's response to extra air, that is why holding your breathe or getting scared suddenly are "cures" for hiccups. By holding or losing air, you are helping your body get back to normal.

Some people say to focus on something else entirely (usually something weird- like pink elephants with blue polka-dots). This works because it takes your mind off off your breathing and allows your body to fix itself naturally. Usually when we get the hiccups we try to control our own breathing to get rid of them, sometimes just making it worse. If you think of something else, you breathing goes back to being the involuntary action it is supposed to be.
