Several thoughts.

Afternoon headaches can be a symptom of hypoglycemia. Do you also experience low energy in the afternoon, or shaking if you get too hungry? If any of this sounds right, you should be sure to eat a high protein breakfast, have a few protein snacks handy to eat when you first feel hungry, if you can't get away to eat a proper meal. Once you have a headache from low blood sugar, you do have to sleep it off. Also, caffeine affects blood sugar. It could be a factor, if your blood sugar is low anyway. I have this problem, but I love coffee, so I have to make sure that I do what I've suggested you do--eat a filling breakfast including a good portion of protein, have snacks handy for mid-morning/mid-afternoon, and never skip lunch or dinner, even if they are light meals. Only then am I able to enjoy my coffee without negatively affecting my blood sugar. There are tests that your doctor can give you to diagnose this, but you can check it yourself by doing the things I've suggested--they are also pretty much the things your doctor would tell you to do if the tests show you are hypoglycemic.

Low blood sugar affects your central nervous system, so it can launch something that feels like a panic attack.

Maybe you're not a hypochondriac--maybe you have hypoglycemia, or a system that requires good meal habits.

Another thought I have is that there is a minor heart valve issue that can create the same symptoms that hypoglycemia does--it's called mitral valve prolapse. That's actually what I have. More women are born with this than men, but you could be one of the men with it. Sometimes the doctor can hear the floppy valve with a stethoscope, but often not. It sometimes has to be diagnosed with an EKG. Talk to your doctor about this--I discovered the link between my reactive blood sugar issues, my headaches, and my panic attacks when I saw an internist who had a subspecialty in cardiology. She put all these things together, put me on a generic med that I've been taking for several years now (that costs me $8/mo.), and I have been utterly symptom free since the first day I took the med. Talk to your doctor about the possibility of this.

Another thought is that mechanical problems with the jaw can cause tension headaches. Do you grind your teeth, day or night? Do you bite the inside of your cheek? Do you tense your jaw? Do you have any problems with your bite (overbite, underbite, crossbite)? Your dentist can check this out.

There's a reason for what you're experiencing. You seem really insightful and have done a really good job of describing your symptoms. Stick with your investigation, and you will figure it out. Headaches are a symptom of so many things that it's tricky sometimes to find the source.
