Question: Can anyone tell me where i can download a self certificate form for a period of a week (5-7days) abcence from work?
is the a standard form i should be using?

These are often available from the workplace and always fromyour GP's surgery. You can download one from :

I dont think you actually need a form- you can self certify for 7 days from work but this is just verbal. Anything after that requires a note from your doctor.
Your workplace should have a form for you to fill in when you get back, if they dont, then one isnt needed for that firm, th Royal Mail has one to fill in, but not everywhere bothers.
In my experience most employers have their own form. Ask HR or personnel.
