Bad breath even after brushing?!
Question: I brush twice daily & brush my tongue and everything.
Yet usually a little while after brushing, I get a terrible taste in my mouth and I can tell I have bad breath.

You could have a bad tooth and the decay is causing the bad odor. Also, if you have really bad allergies, you know whatever is in your sinuses drains down the back of your throat and that can cause bad breath, regardless of how clean your teeth are.
Also, tho, you don't mention if you're flossing or not. You can brush your teeth but if you don't floss to get the food left between the teeth that can cause problems as well.
See a dental hygienist. You may have plaque buildup or something worse.
Maybe you have an infected tooth. Do you floss, rinse with mouthwash, brush, gargle, etc, etc, and still have the bad breath feeling? If so maybe you should see your dentist. I know a rotten tooth can make your whole mouth feel gross.
do you get these nasty little white things in your tonsils? they cause bad breath. i get them. ugh.
I am not sure what it is called but there is a condition in which odor comes from your stomach. I knew a girl who had it and the odor would fill a whole room. No amount of mouth rinse or brushing would help. It was an "internal" problem and she had to see a physician who prescribed medication. After that she had no problems.
