Answers:
If you are in Canada, there is a medical part of employment insurance!. See if your employer will give you an unpaid leave from work first, and that EI may cover some monetary loss!.

For a medical EI, you need to have at least 600 insured hours of work in the last 52 weeks!. You will need to get a record of employment from your employer to support that!. (if they put leave of absence in the reason for separation, ask them to put why in the comments; or ask them to put reason for leaving as sickness)

You will also need a letter from your doctor to support your claim for sickness leave!. Try and get your doctor to say you need to be off work for at least 15 weeks!. That is the maximum allowed for sickness EI, and that way you wont have to go back to the doctor after a few weeks to get ANOTHER doctor's note for EI!. The EI process takes long enough for people anyways, without that extra bit of stress in there!.

Here is from the site:
Sickness benefits
Sickness benefits may be paid up to 15 weeks to a person who is unable to work because of sickness, injury or quarantine!. To receive sickness benefits you are required to have worked for 600 hours in the last 52 weeks or since your last claim!. A medical certificate must be obtained to confirm the duration of your incapacity!. The fees requested by your doctor or dentist are entirely at your own expense!.

A person who makes a claim for sickness benefits is not only required to prove to be unable to work but also that he or she would be otherwise available for work!.
