Brain aneurysms are generally not painful (and often not even symptomatic) at all until they rupture. At rupture, the pain can be extremely variable depending on the location of aneurysm. Many describe it as a very sudden and severe headache. "The worst headache of their life." The pain subsides but the damage can be substantial. The area of the brain downstream from the aneurysm is starved for blood (stroke).
