Your ears are probably waxy. Clean them out with a cotton swab and you should be good to go. Wet wash clothes held over your ears help too. Or a wet paper towel.

Sometimes fluid gets trapped behind your eardrum. Allergies, colds, and/or swimming can cause this.

Also, after your done taking the antibiotic, go to an Ear, Nose and Throat specialist. Have him clean the wax out of your ears. It doesn't hurt, just feels a bit odd at first..
Wax can trap water in your ears also. It causes a lot of problems sometimes.
These doctors have a tiny camera that is connected to a tube, and can show you what's in your ear, if your interested. Don't worry, all you usually see is wax...

You're right, Q tips are for just the outside of the ear, the part you can see. Don't insert inside the ear, it'll push the wax in farther. Or you could hurt your eardrum.
