The worst is death. If it is bad enough you need surgery.
You can get anemic from blood loss
You can have some major reaction from stomach acid getting to the blood stream, making its pH go down and messing up the whole body
You can get some infection from acid-resistant bacteria
And...of course, die.
Depending of the size, medications can solve the problem. Otherwise, surgery usually does it. It'd be really some ultimate lack of luck if the surgery doesn't work, since you can literally take chunks ou of the stomach (surgically) without much problem, considering those stomach-reducing surgeries to fight back obesity(wich IS a disease).
