You will automatically be on Part A at 65 but you don't have to accept Part B, although there may be a penalty if you don't. If you're currently working and have group health insurance you won't get a penalty, but you have to sign up within 8 months after your group policy terminates. If you are retired but still getting employer sponsored health it is secondary to medicare and you will have a penalty.

If you are not working you still want to get the Part D prescription drug coverage because there are penalties for not signing up when eligible.

You'll want to visit a local independent agent that deals with senior issues for more a exact answer for your situation. You may find that you're better off with the Part B and a supplement or Medicare Advantage plan.
