However, I think doctors should be allowed to do it, since if they want it bad enough, they'll just hurt themselves anyway (shoot it with a shotgun, leave it in dry ice for hours until it's frostbitten, etc). At least make it safe.

Who would want to be an amputee though? I mean, I just don't see the pleasure in losing a limb. But if people actually want to do it, I agree that it's best to make it as safe as possible, considering that the other two options you've mentioned are much more dangerous than a medical procedure, but if somebody wanted to be an amputee badly enough, I can see them trying the other options.
