Anyone know of a medical quiz type website?

I am looking for a website that will help me list everything that is going on so I know and remember everything to tell my doctor tomarrow at my appointment. thanks for any help.

a website that will help me list all my symptoms and help me recognise the things that are symptoms that i didn't think were, kind of like a check list or somthing.
