It varies, depending on how bad the situation in the appendix is. You can suffer from chronic appendicitis for years, before it finally gets to the point surgery is needed. Some folks are sick for days before it becomes obvious that it's the appendix causing the problem, and sometimes it will just flare up out of nowhere. If you are running fever, along with incredible right sided pain, halfway between the belly button and the right hip joint, with or without diarrhea, feel nauseated, or otherwise feel like something is wrong, then it's a good possibility that's the trouble. It can stew for a day or so, and then rapidly go downhill in hours. I had a questionable appendix for almost a year, then I was sick for three days before the fever spiked and we parted company. But I've also had patients that woke up fine in the morning, took ill around lunchtime, and were in surgery by dinner. It's just rather variable, too much to predict a matter of time.
