As a healthcare provider this is what I think. There is different kind of nerves for pain...the kind that is specific to the area...arm, leg, and there is viseral pain that is in the organs inside. Viseral pain is not so specific for where the pain is originating from and tends to be diffuse. Example: your appendix is on the lower right side of your abdomen. But severe appendicitis can cause pain around the bellybutton. I think the testicles being in pain is more of a viseral pain....can be more nonspecific and sprend to pain in areas that were not hurt.
