If someone's left butt cheek is bigger than the right can that mean their feet are different sizes?

If you know the difference between the two sides of your derrière, which is on the back, but are not sure about a difference in the size of your feet, which are in plain view, I would rule that out as a cause. I know of no such link anyway, unless one leg grew larger than the other as the result of a birth defect or circulatory problem.

You may be involved in a sport or activity that overdevelops the muscles in one leg. For example, a sprinter who does 200 meters or longer has to take the curve in a counter-clockwise direction. This causes the right glute muscles to work harder to swing the right leg farther on the outside of that curve. You would see other development in that leg, though, not just the glute.

If one of your feet is indeed larger than the other, any running activity could cause the muscles to work harder on that side to pull the extra weight. If that is the case, adding weight to the other shoe would equalize development, although it would likely slow you down.

If you had a job that involved repetitive motion such as stepping up on a ladder or platform, and you always went up on the same leg, you could overdevelop that muscle, too.

Another possibility is a circulatory problem. Lots of people have a wallet or something else in a back pocket on one side, cutting off circulation on that side when they sit. This wouldn't matter too much unless you are sitting for extended periods of time, like a professional driver, etc. Sleep habits could be another thing to look at, since many people gravitate toward the fetal position during sleep.

Some people are born with congenital defects that cause the pelvis to skew asymmetrically, which could make equally developed glute muscles appear differently. This would be revealed by a series of x-rays from different angles, or by examination from a doctor, chiropractor, or neuromuscular therapist. If that is the case, it could be corrected by extended therapy or surgery.

Another possibility involving bone development is a "leg length discrepancy," which affects roughly 56% of the population. A difference of as little as half a centimeter between the length of your legs could have a profound effect on your development and health, including chronic pain, scoliosis, TMJ, and headaches, as well as muscular asymmetry.

In any case, unless you are involved in some of those one-sided activities I mentioned, you might want to have it examined. There could be something else deep in the complex muscular structure of the hip that needs professional attention.
