It's always tough, but you have to understand that having a fear of needles is all in your head. The feeling of a needle stick is mildly unpleasant, but goes away quite quickly. With a shot, you might be left with a bruise, but I can't imagine that you haven't gotten bruises in your life before and been just fine. So really, you have to believe that the act itself is pretty small, not as bad as you've now built it up to in your mind. It can also help to focus on your breathing while you're in the office, and as you feel yourself getting nervous think about something that you enjoy that will take your mind off of it. Bring a magazine in of your favorite sport? Let the nurses know, and they will be supportive. It's going to be tough anyway, because phobias are quite difficult to get over, but try your best, the worse that will happen is that you'll get through it- which ain't that bad!
