After about 30 days you should be able to pass a blood and urine test. However, some companies are now starting to do a hair test. Nicotine stays in the hair follicles until it grows out and is cut off.

Most companies will give you a non-smoker rate after quitting for 1 year. It's not worth it to lie on your application because if you die the insurance company can rescind the policy if they find out.
