If I am ever in an accident or suffer some trauma where I would need 24/7 life support to live, I would rather be allowed to die. How do I get a do not resuscitate card for my wallet?

Answers:

Type the name of your state and "do not resuscitate" into Google and a number of forms should come up. This is what I did to find one for Florida.
