Short Term Disability - H 1 B Condidate - Fall sick in India during leave?

Here is the case:
1. Honey is working in US on H 1 B and covered under short term disability plan.
2. He went to India on 2 week leave and fell sick and hospitalized for 4 week.
3. Came back and start working again.
4. Is he elligible to claim short term disability?
5. If yes - what all document are required? and What is the procedure?
