The injured Ranger's femoral artery was severed. It's the major artery supplying the leg with blood. Arteries are muscular and can contract into a wound cavity when traumatized or torn. That's what happened in the book/flick. The medic was attempting to pull the retracted artery out of the wound, to clamp it shut with some forceps.
Now military medics are equipped with blood-clotting powders and compresses, that almost immediately stop hemorraging (heavy bleeding), even high-pressure arterial bleeding. Sadly, it appears that stuff wasn't yet invented in 1993.
